There are programs offered in or near Iliamna which service both methadone and buprenorphine clients only, drugs that are administered in what is called opioid maintenance therapy. Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, meaning the same neural receptors that make a person feel high from morphine or heroin are stimulated by these two drugs so that an opioid dependent client will ideally not become involved in drug seeking behavior and won't experience cravings and withdrawal. There are dangers with side effects, drug interactions, and even overdose with both buprenorphine and methadone, which is why they are highly controlled and can only be administered in a clinical setting.
Buprenorphine and methadone clients take these medications daily so they don't experience cravings, withdrawal, etc. but ideally at a dose which isn't also making them feel euphoria. Euphoria occurs at at higher dosages, more significantly with methadone than buprenorphine. At clinics that treat methadone and buprenorphine clients only, treatment therapies are generally limited beyond managing the medication and the primary treatment goal is to handle the physical symptoms of their dependence. Some centers do deliver a small amount of counseling and group therapy as secondary treatment, and this is encouraged.
