Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, meaning the same neural receptors which make a person feel high from heroin or prescription pain killers are activated by these two drugs so therefore an opioid dependent client will instead not engage in drug seeking behavior and won't experience cravings and withdrawal. Some clinics do provide a limited amount of counseling and therapy as supplementary treatment, and it is recommended and encouraged.
