Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, meaning the same neural receptors which make a person feel high from morphine or heroin are activated by these two drugs so that an opiate dependent client will ideally not become involved in addict behavior and won't experience withdrawal symptoms. There are dangers with side effects, drug interactions, and even addiction and overdose with both methadone and buprenorphine, which is why they are heavily controlled and can only be administered in a clinical setting.

Some programs do provide a limited amount of counseling and group therapy as supplementary services, and this is recommended and encouraged.
