Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, meaning the same nerve cell receptors which cause an individual to feel high from morphine or heroin are stimulated by these two drugs so that an opioid dependent client will ideally not engage in addict behavior and won't experience withdrawal symptoms. There are risks of side effects, drug interactions, and even overdose with both methadone and buprenorphine, which is why they are heavily controlled and can only be prescribed in a clinical setting.

Methadone and buprenorphine clients are on these medications daily so they don't experience cravings, withdrawal, etc. but ideally at a dosage which isn't also making them feel euphoria. Euphoria can be experienced at at higher dosages, more so with methadone than buprenorphine. At clinics that treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are generally limited beyond this and the primary treatment goal is to treat the physical symptoms of their dependence. Some programs do deliver a limited amount of counseling and group therapy as supplementary services, and it is recommended and encouraged.
