There are addiction programs offered in or near Madison that cater to both methadone and buprenorphine clients only, controlled medications that are dispensed in what is called opioid maintenance therapy. Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, which means the same nerve cell receptors that cause an individual to feel high from heroin or prescription pain killers are activated by these two drugs so that an opiate dependent client will ideally no longer become involved in drug seeking behavior and won't have withdrawal symptoms. There are risks of side effects, drug interactions, and even addiction and overdose with both methadone and buprenorphine, which is why they are highly regulated and can only be administered in a clinical setting.

Buprenorphine and methadone clients are on these medications daily so they don't experience withdrawal symptoms, etc. but hopefully at a dosage which isn't at the same time causing them to feel euphoria. Euphoria can be experienced at a high dosage, more so with methadone than buprenorphine. At programs that treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are limited beyond this and the main treatment objective is to treat the withdrawal symptoms and put an end to the illicit drug use. Some programs do deliver a small amount of counseling and group therapy as secondary treatment, and it is encouraged.
