There are substance abuse programs offered in or near Hawaii National Park which service both methadone and buprenorphine clients only, medications which are used in what is called opioid maintenance therapy. Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, which means the same neural receptors which make a person feel high from heroin or prescription pain killers are stimulated by these two drugs so therefore an opiate addicted client will ideally not engage in addict behavior and won't experience withdrawal symptoms. There are dangers with side effects, drug interactions, and even overdose with both methadone and buprenorphine, which is why they are highly controlled and can only be dispensed in a clinical setting.

Buprenorphine and methadone clients take these medications daily so they don't experience withdrawal symptoms, etc. but ideally at a dosage which isn't at the same time causing them to feel high. Euphoria can be experienced at a high dosage, more significantly with methadone than buprenorphine. At centers that treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are limited beyond this and the main treatment goal is to treat the withdrawal symptoms and put an end to the illicit drug use. Some centers do offer a limited amount of counseling and group therapy as secondary treatment, and it is recommended and encouraged.
