Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, meaning the same nerve cell receptors that make a person feel high from morphine or heroin are stimulated by these two drugs so that an opiate addicted client will ideally no longer become involved in addict behavior and won't experience withdrawal symptoms. There are risks of drug interactions, side effects, and even addiction and overdose with both buprenorphine and methadone, which is why they are heavily controlled and can only be administered in a clinical setting.

Methadone and buprenorphine clients take these medications consistently so they don't experience cravings, withdrawal, etc. but hopefully at a dosage which isn't at the same time causing them to feel euphoria. Euphoria is possible at a high dosage, more so with methadone than buprenorphine. At centers that treat methadone and buprenorphine clients only, treatment therapies are limited beyond managing the medication and the primary treatment goal is to handle the withdrawal symptoms and put an end to the illicit drug use. Some centers do provide a small amount of counseling and group therapy as secondary services, and it is recommended and encouraged.
