There are programs available in or near Nashville that cater to both methadone and buprenorphine clients only, controlled medications which are used in what is called opioid maintenance therapy. Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, meaning the same nerve cell receptors which make a person feel high from morphine or heroin are stimulated by these two drugs so therefore an opioid dependent client will ideally no longer become involved in addict behavior and won't experience withdrawal symptoms. There are dangers with side effects, drug interactions, and even addiction and overdose with both buprenorphine and methadone, which is why they are highly controlled and can only be dispensed in a clinical setting.

Methadone and buprenorphine clients take these medications regularly so they don't experience withdrawal symptoms, etc. but ideally at a dose which isn't at the same time causing them to feel euphoria. At programs that treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are generally limited beyond this and the primary treatment goal is to address the physical symptoms of their dependence. Some centers do provide a small amount of counseling and group therapy as supplementary treatment, and this is encouraged.
