There are programs offered in or near Pendleton which service both methadone and buprenorphine clients only, drugs which are administered in what is known as opioid maintenance therapy. Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, meaning the same nerve cell receptors which cause an individual to feel high from heroin or prescription pain killers are stimulated by these two drugs so that an opiate dependent client will ideally no longer engage in drug seeking behavior and won't have withdrawal symptoms. There are dangers with drug interactions, side effects, and even overdose with both methadone and buprenorphine, which is why they are heavily controlled and can only be dispensed in a clinical setting.

Methadone and buprenorphine clients are on these medications daily so they don't experience withdrawal symptoms, etc. but ideally at a dose which isn't at the same time causing them to feel euphoria. Euphoria can be experienced at at higher dosages, more so with methadone than buprenorphine. At programs which treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are generally limited beyond managing the medication and the primary treatment objective is to manage the physical symptoms of their dependence. Some programs do provide a small amount of counseling and group therapy as secondary treatment, and it is encouraged.
