Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, which means the same neural receptors that make a person feel high from heroin or prescription pain killers are activated by these two drugs so that an opioid dependent client will instead no longer become involved in addict behavior and won't experience cravings and withdrawal. There are risks of side effects, drug interactions, and even addiction and overdose with both buprenorphine and methadone, which is why they are highly regulated and can only be administered in a clinical setting.

At centers which treat methadone and buprenorphine clients only, treatment therapies are limited beyond this and the main treatment goal is to handle the withdrawal symptoms and put an end to the illicit drug use. Some centers do provide a small amount of counseling and group therapy as supplementary treatment, and it is encouraged.
