Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, meaning the same nerve cell receptors which cause an individual to feel high from morphine or heroin are stimulated by these two drugs so therefore an opioid dependent client will instead no longer become involved in drug seeking behavior and won't have withdrawal symptoms. There are risks of side effects, drug interactions, and even overdose with both buprenorphine and methadone, which is why they are heavily controlled and can only be prescribed in a clinical setting.

Methadone and buprenorphine clients take these medications consistently so they don't experience withdrawal symptoms, etc. but ideally at a dose which isn't at the same time causing them to feel euphoria. Euphoria is possible at at higher dosages, more so with methadone than buprenorphine. At clinics that treat methadone and buprenorphine clients only, treatment therapies are limited beyond managing the medication and the primary treatment objective is to handle the physical symptoms of their dependence. Some centers do provide a limited amount of counseling and group therapy as auxiliary services, and this is recommended and encouraged.
