There are substance abuse programs available in or near Cornell which service both methadone and buprenorphine clients only, controlled medications which are administered in what is known as opioid maintenance therapy. Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, which means the same nerve cell receptors that make a person feel high from morphine or heroin are activated by these two drugs so therefore an opiate dependent client will ideally not become involved in addict behavior and won't have cravings and withdrawal. There are dangers with side effects, drug interactions, and even overdose with both methadone and buprenorphine, which is why they are highly controlled and can only be prescribed in a clinical setting.

Buprenorphine and methadone clients are on these medications regularly so they don't experience cravings, withdrawal, etc. but hopefully at a dose which isn't also making them feel euphoria. Euphoria is possible at at higher dosages, more so with methadone than buprenorphine. At clinics which treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are limited beyond this and the main treatment objective is to manage the physical symptoms of their dependence. Some programs do offer a limited amount of counseling and group therapy as auxiliary treatment, and this is encouraged.
