There are substance abuse programs available in or near Moscow that cater to both methadone and buprenorphine clients only, controlled medications which are used in what is known as opioid maintenance therapy. Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, meaning the same neural receptors which make a person feel high from heroin or prescription pain killers are activated by these two drugs so therefore an opiate dependent client will instead no longer become involved in drug seeking behavior and won't have withdrawal symptoms. There are dangers with side effects, drug interactions, and even overdose with both buprenorphine and methadone, which is why they are highly controlled and can only be prescribed in a clinical setting.

Buprenorphine and methadone clients are on these medications regularly so they don't experience withdrawal symptoms, etc. but ideally at a dose which isn't also making them feel euphoria.
