Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, which means the same neural receptors that make a person feel high from morphine or heroin are stimulated by these two drugs so therefore an opioid addicted client will ideally no longer engage in addict behavior and won't experience withdrawal symptoms. There are dangers with side effects, drug interactions, and even overdose with both buprenorphine and methadone, which is why they are heavily controlled and can only be administered in a clinical setting.

Some clinics do offer a limited amount of counseling and group therapy as secondary treatment, and this is recommended and encouraged.
