There are programs offered in or near Richmond which service both methadone and buprenorphine clients only, controlled medications that are dispensed in what is called opioid maintenance therapy. Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, which means the same nerve cell receptors that make a person feel high from morphine or heroin are activated by these two drugs so therefore an opiate dependent client will instead not become involved in drug seeking behavior and won't experience cravings and withdrawal. There are risks of side effects, drug interactions, and even addiction and overdose with both buprenorphine and methadone, which is why they are highly controlled and can only be dispensed in a clinical setting.
Methadone and buprenorphine clients take these medications regularly so they don't experience cravings, withdrawal, etc. but hopefully at a dosage which isn't also causing them to feel high. Euphoria is possible at a high dosage, more significantly with methadone than buprenorphine. At clinics that treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are limited beyond this and the main treatment goal is to manage the withdrawal symptoms and put an end to the illicit drug use. Some programs do offer a limited amount of counseling and therapy as auxiliary treatment, and this is recommended and encouraged.
