Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, meaning the same neural receptors that cause an individual to feel high from heroin or prescription pain killers are stimulated by these two drugs so therefore an opioid addicted client will ideally not engage in drug seeking behavior and won't have cravings and withdrawal. There are dangers with side effects, drug interactions, and even overdose with both methadone and buprenorphine, which is why they are heavily controlled and can only be administered in a clinical setting.
Buprenorphine and methadone clients take these medications daily so they don't experience cravings, withdrawal, etc. but hopefully at a dosage which isn't at the same time causing them to feel euphoria. Euphoria is possible at at higher dosages, more so with methadone than buprenorphine. At centers that treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are limited beyond this and the primary treatment goal is to treat the physical symptoms of their dependence. Some clinics do provide a limited amount of counseling and therapy as auxiliary treatment, and this is recommended and encouraged.
