There are programs offered in or near St. Louis that cater to both methadone and buprenorphine clients only, controlled medications that are used in what is called opioid maintenance therapy. Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, meaning the same neural receptors that cause an individual to feel high from morphine or heroin are activated by these two drugs so therefore an opioid dependent client will instead no longer become involved in drug seeking behavior and won't have withdrawal symptoms. There are risks of drug interactions, side effects, and even overdose with both methadone and buprenorphine, which is why they are heavily regulated and can only be dispensed in a clinical setting.

Methadone and buprenorphine clients take these medications daily so they don't experience cravings, withdrawal, etc. but hopefully at a dose which isn't also making them feel euphoria. Euphoria is possible at a high dosage, more significantly with methadone than buprenorphine. At programs which treat methadone and buprenorphine clients only, treatment therapies are generally limited beyond managing the medication and the primary treatment objective is to manage the withdrawal symptoms and put an end to the illicit drug use. Some clinics do deliver a limited amount of counseling and group therapy as supplementary treatment, and this is recommended and encouraged.
