There are substance abuse programs offered in or near Fulda which service both methadone and buprenorphine clients only, drugs which are dispensed in what is called opioid maintenance therapy. Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, meaning the same neural receptors which make a person feel high from morphine or heroin are stimulated by these two drugs so therefore an opioid dependent client will instead not engage in drug seeking behavior and won't experience withdrawal symptoms. There are dangers with side effects, drug interactions, and even addiction and overdose with both buprenorphine and methadone, which is why they are highly regulated and can only be dispensed in a clinical setting.
Methadone and buprenorphine clients are on these medications daily so they don't experience withdrawal symptoms, etc. but hopefully at a dosage which isn't at the same time making them feel high. Euphoria can be experienced at at higher dosages, more so with methadone than buprenorphine. At centers that treat methadone and buprenorphine clients only, treatment therapies are limited beyond managing the medication and the primary treatment objective is to manage the withdrawal symptoms and put an end to the illicit drug use. Some centers do offer a small amount of counseling and therapy as secondary services, and this is encouraged.
