Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, meaning the same nerve cell receptors which cause an individual to feel high from morphine or heroin are stimulated by these two drugs so therefore an opioid dependent client will instead no longer become involved in drug seeking behavior and won't have withdrawal symptoms. There are risks of drug interactions, side effects, and even overdose with both methadone and buprenorphine, which is why they are heavily regulated and can only be dispensed in a clinical setting.
Methadone and buprenorphine clients are on these medications daily so they don't experience withdrawal symptoms, etc. but ideally at a dosage which isn't also causing them to feel high. Euphoria is possible at a high dosage, more significantly with methadone than buprenorphine. At centers which treat methadone and buprenorphine clients only, treatment therapies are limited beyond managing the medication and the main treatment goal is to address the physical symptoms of their dependence. Some clinics do deliver a limited amount of counseling and group therapy as auxiliary treatment, and it is recommended and encouraged.
