Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, meaning the same neural receptors that make a person feel high from heroin or prescription pain killers are activated by these two drugs so therefore an opioid addicted client will ideally no longer engage in addict behavior and won't experience cravings and withdrawal. There are dangers with side effects, drug interactions, and even overdose with both buprenorphine and methadone, which is why they are heavily regulated and can only be administered in a clinical setting.
Methadone and buprenorphine clients take these medications daily so they don't experience cravings, withdrawal, etc. but ideally at a dose which isn't also making them feel euphoria. Euphoria can be experienced at a high dosage, more significantly with methadone than buprenorphine. At clinics which treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are generally limited beyond managing the medication and the main treatment objective is to handle the physical symptoms of their dependence. Some programs do offer a small amount of counseling and group therapy as supplementary services, and it is encouraged.
