Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, meaning the same nerve cell receptors that make a person feel high from morphine or heroin are stimulated by these two drugs so therefore an opioid dependent client will ideally no longer become involved in addict behavior and won't have withdrawal symptoms. There are risks of side effects, drug interactions, and even addiction and overdose with both buprenorphine and methadone, which is why they are heavily regulated and can only be prescribed in a clinical setting.

Buprenorphine and methadone clients are on these medications regularly so they don't experience withdrawal symptoms, etc. but hopefully at a dosage which isn't at the same time causing them to feel euphoria. Euphoria is possible at at higher dosages, more so with methadone than buprenorphine. At centers which treat methadone and buprenorphine clients only, treatment therapies are generally limited beyond managing the medication and the primary treatment goal is to manage the withdrawal symptoms and put an end to the illicit drug use. Some clinics do provide a limited amount of counseling and group therapy as secondary treatment, and it is recommended and encouraged.
