There are recovery programs available in or near Philadelphia which service both methadone and buprenorphine clients only, controlled medications which are dispensed in what is known as opioid maintenance therapy. Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, meaning the same neural receptors that cause an individual to feel high from morphine or heroin are activated by these two drugs so that an opiate dependent client will ideally no longer engage in drug seeking behavior and won't have cravings and withdrawal. There are risks of drug interactions, side effects, and even overdose with both methadone and buprenorphine, which is why they are heavily regulated and can only be prescribed in a clinical setting.

Buprenorphine and methadone clients are on these medications regularly so they don't experience withdrawal symptoms, etc. but hopefully at a dose which isn't at the same time making them feel high. Euphoria is possible at a high dosage, more significantly with methadone than buprenorphine. At programs which treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are generally limited beyond managing the medication and the primary treatment goal is to treat the withdrawal symptoms and put an end to the illicit drug use. Some programs do provide a limited amount of counseling and group therapy as auxiliary treatment, and this is recommended and encouraged.
