Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, meaning the same neural receptors that make a person feel high from morphine or heroin are activated by these two drugs so that an opiate dependent client will ideally no longer engage in drug seeking behavior and won't experience withdrawal symptoms. There are dangers with drug interactions, side effects, and even overdose with both buprenorphine and methadone, which is why they are heavily controlled and can only be dispensed in a clinical setting.

Buprenorphine and methadone clients take these medications daily so they don't experience cravings, withdrawal, etc. but hopefully at a dosage which isn't also causing them to feel euphoria. At clinics that treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are limited beyond managing the medication and the main treatment goal is to address the physical symptoms of their dependence. Some clinics do provide a small amount of counseling and group therapy as auxiliary treatment, and it is encouraged.
