Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, which means the same neural receptors that make a person feel high from heroin or prescription pain killers are activated by these two drugs so that an opiate dependent client will ideally not become involved in addict behavior and won't have cravings and withdrawal. There are risks of side effects, drug interactions, and even addiction and overdose with both methadone and buprenorphine, which is why they are heavily controlled and can only be prescribed in a clinical setting.

At programs which treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are generally limited beyond this and the primary treatment goal is to treat the withdrawal symptoms and put an end to the illicit drug use. Some programs do offer a small amount of counseling and therapy as secondary treatment, and this is recommended and encouraged.
