Vivitrol is a medication used to help prevent drug or alcohol abuse and relapse, and is used specifically for alcohol and opioid drugs such as heroin and prescription medications. Vivitrol is an extended release type of the medication Naltrexone, which one can get via a monthly injection. It isn't addictive, and is what is known as an antagonist. When an individual takes Vivitrol, it cuts off the receptors that would usually be triggered by alcohol or opioids, so when someone drinks or tries to get high on opioids they essentially can't get drunk or high.

First off, an individual who intends to start Vivitrol needs to have been off of opioids for a minimum of 7-10 days. This includes clients who are participating in opioid maintenance therapy and are taking meds such as Buprenorphine or Methadone. For a heroin abuser or someone who has been on Methadone for several years, abrupt cessation can be very difficult, but withdrawal is something they will have to go through for a minimum of a week if they want to start Vivitrol.
