It is estimated that around 75% of clients at residential rehab centers for drug addiction and alcoholism are smokers. While it is a fact that nicotine is an addictive substance and people in rehab for addiction should strive to be free from all addictive substances, this can present problems that make enforcing a non-smoking facility outweigh the benefits of this restriction. A person who is addicted to a substance that could very well kill them immediately might not agree to get help if they must quit smoking cigarettes, so many rehab centers in Indian allow smoking with some limitations, restrictions and in designated areas of their facility.

Some programs permit clients to smoke anywhere, so long as it is outside for example. Some facilities have a designated smoking area, and this is the only place that clients can smoke. Because nicotine is addictive, even treatment programs that have a designated area and allow smoking typically offer alternatives to smoking if the client wants to quit, such as nicotine replacement gum, patches and prescriptions. There are also rehab centers that permit smoking in designated areas, but provide education on the health risks associated with smoking by including this into their program either by way of direct one on one counseling or by providing informational literature for clients to consider. In programs which allow smoking in designated areas this is sometimes only authorized for clients, whereas staff are sometimes restricted from smoking at the center or are asked to not smoke in front of clients.

Publicly funded substance abuse treatment centers are bound by the rules and restrictions in regards to smoking that are enforced by state and federal agencies which oversee and administer these programs. Private rehabilitation centers have a bit more leeway and don't have as many restrictions imposed on them, so they be more likely to offer designated areas for smoking.
