Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, meaning the same neural receptors that cause an individual to feel high from morphine or heroin are activated by these two drugs so that an opioid dependent client will ideally not become involved in drug seeking behavior and won't experience cravings and withdrawal. There are risks of drug interactions, side effects, and even addiction and overdose with both methadone and buprenorphine, which is why they are heavily controlled and can only be prescribed in a clinical setting.

At programs which treat methadone and buprenorphine clients only, treatment therapies are generally limited beyond this and the primary treatment goal is to manage the withdrawal symptoms and put an end to the illicit drug use. Some centers do offer a small amount of counseling and group therapy as secondary treatment, and it is encouraged.
