There are substance abuse programs available in or near Harvard which service both methadone and buprenorphine clients only, medications which are dispensed in what is called opioid maintenance therapy. Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, which means the same nerve cell receptors that cause an individual to feel high from heroin or prescription pain killers are stimulated by these two drugs so therefore an opioid dependent client will instead no longer engage in drug seeking behavior and won't have cravings and withdrawal. There are risks of side effects, drug interactions, and even overdose with both methadone and buprenorphine, which is why they are heavily regulated and can only be dispensed in a clinical setting.

Methadone and buprenorphine clients are on these medications regularly so they don't experience cravings, withdrawal, etc. but hopefully at a dose which isn't also making them feel high. Euphoria is possible at a high dosage, more significantly with methadone than buprenorphine. At programs which treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are limited beyond this and the primary treatment goal is to treat the withdrawal symptoms and put an end to the illicit drug use. Some clinics do provide a small amount of counseling and therapy as secondary services, and this is encouraged.
