There are addiction programs offered in or near Moscow which service both methadone and buprenorphine clients only, drugs which are dispensed in what is known as opioid maintenance therapy. Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, meaning the same neural receptors that make a person feel high from heroin or prescription pain killers are activated by these two drugs so therefore an opioid dependent client will instead not engage in drug seeking behavior and won't experience withdrawal symptoms. There are risks of side effects, drug interactions, and even overdose with both buprenorphine and methadone, which is why they are highly regulated and can only be prescribed in a clinical setting.

Methadone and buprenorphine clients take these medications consistently so they don't experience cravings, withdrawal, etc. but hopefully at a dose which isn't at the same time making them feel euphoria. Euphoria is possible at a high dosage, more significantly with methadone than buprenorphine. At centers that treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are generally limited beyond this and the main treatment goal is to manage the withdrawal symptoms and put an end to the illicit drug use. Some programs do offer a limited amount of counseling and group therapy as supplementary services, and this is recommended and encouraged.
