Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, meaning the same nerve cell receptors which make a person feel high from morphine or heroin are stimulated by these two drugs so that an opioid addicted client will ideally no longer become involved in addict behavior and won't experience withdrawal symptoms. There are dangers with drug interactions, side effects, and even overdose with both buprenorphine and methadone, which is why they are highly controlled and can only be administered in a clinical setting.

Methadone and buprenorphine clients are on these medications regularly so they don't experience withdrawal symptoms, etc. but hopefully at a dosage which isn't at the same time making them feel high. Euphoria can be experienced at a high dosage, more so with methadone than buprenorphine. At programs that treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are limited beyond managing the medication and the main treatment objective is to handle the physical symptoms of their dependence. Some programs do provide a small amount of counseling and group therapy as auxiliary services, and it is encouraged.
