There are risks of drug interactions, side effects, and even addiction and overdose with both methadone and buprenorphine, which is why they are highly controlled and can only be dispensed in a clinical setting.
Buprenorphine and methadone clients are on these medications consistently so they don't experience cravings, withdrawal, etc. but hopefully at a dose which isn't at the same time making them feel euphoria. Euphoria is possible at at higher dosages, more so with methadone than buprenorphine. At centers which treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are limited beyond managing the medication and the primary treatment objective is to treat the physical symptoms of their dependence. Some clinics do provide a limited amount of counseling and therapy as supplementary services, and this is encouraged.
