There are addiction programs available in or near Detroit which service both methadone and buprenorphine clients only, controlled medications which are used in what is known as opioid maintenance therapy. Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, meaning the same nerve cell receptors which cause an individual to feel high from heroin or prescription pain killers are activated by these two drugs so that an opioid dependent client will ideally no longer engage in drug seeking behavior and won't have cravings and withdrawal. There are risks of drug interactions, side effects, and even addiction and overdose with both buprenorphine and methadone, which is why they are heavily controlled and can only be dispensed in a clinical setting.

Buprenorphine and methadone clients are on these medications regularly so they don't experience withdrawal symptoms, etc. but ideally at a dose which isn't also making them feel euphoria. Euphoria can be experienced at a high dosage, more so with methadone than buprenorphine. At centers which treat methadone and buprenorphine clients only, treatment therapies are generally limited beyond managing the medication and the main treatment goal is to address the physical symptoms of their dependence. Some centers do provide a small amount of counseling and therapy as secondary services, and this is encouraged.
