Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, which means the same nerve cell receptors that make a person feel high from morphine or heroin are activated by these two drugs so that an opioid dependent client will instead not become involved in addict behavior and won't experience withdrawal symptoms. There are risks of side effects, drug interactions, and even addiction and overdose with both buprenorphine and methadone, which is why they are highly regulated and can only be administered in a clinical setting.

Methadone and buprenorphine clients take these medications regularly so they don't experience withdrawal symptoms, etc. but hopefully at a dosage which isn't also making them feel euphoria. Euphoria occurs at a high dosage, more so with methadone than buprenorphine. At clinics that treat methadone and buprenorphine clients only, treatment therapies are generally limited beyond this and the primary treatment goal is to treat the withdrawal symptoms and put an end to the illicit drug use. Some centers do deliver a limited amount of counseling and group therapy as secondary treatment, and it is recommended and encouraged.
