Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, meaning the same nerve cell receptors which make a person feel high from heroin or prescription pain killers are activated by these two drugs so that an opioid addicted client will ideally no longer engage in drug seeking behavior and won't have cravings and withdrawal. There are risks of side effects, drug interactions, and even addiction and overdose with both buprenorphine and methadone, which is why they are highly controlled and can only be administered in a clinical setting.

Methadone and buprenorphine clients are on these medications consistently so they don't experience cravings, withdrawal, etc. but ideally at a dosage which isn't also making them feel euphoria. Euphoria is possible at at higher dosages, more so with methadone than buprenorphine. At clinics which treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are generally limited beyond this and the primary treatment objective is to manage the physical symptoms of their dependence. Some centers do provide a limited amount of counseling and therapy as supplementary treatment, and it is recommended and encouraged.
