Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, which means the same neural receptors which make a person feel high from heroin or prescription pain killers are stimulated by these two drugs so that an opiate dependent client will ideally no longer engage in addict behavior and won't have cravings and withdrawal. At clinics that treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are limited beyond managing the medication and the primary treatment goal is to address the physical symptoms of their dependence. Some clinics do deliver a small amount of counseling and group therapy as auxiliary services, and it is encouraged.
