Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, meaning the same neural receptors which make a person feel high from heroin or prescription pain killers are activated by these two drugs so that an opiate addicted client will ideally not become involved in addict behavior and won't experience cravings and withdrawal. There are dangers with side effects, drug interactions, and even addiction and overdose with both methadone and buprenorphine, which is why they are highly regulated and can only be prescribed in a clinical setting.
Buprenorphine and methadone clients take these medications consistently so they don't experience withdrawal symptoms, etc. but hopefully at a dose which isn't also causing them to feel euphoria. Euphoria occurs at at higher dosages, more so with methadone than buprenorphine. At centers which treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are limited beyond managing the medication and the primary treatment goal is to treat the withdrawal symptoms and put an end to the illicit drug use. Some programs do provide a limited amount of counseling and group therapy as supplementary treatment, and this is encouraged.
