There are substance abuse programs offered in or near Presidio which service both methadone and buprenorphine clients only, medications which are used in what is called opioid maintenance therapy. Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, meaning the same nerve cell receptors that make a person feel high from heroin or prescription pain killers are stimulated by these two drugs so that an opiate addicted client will instead no longer become involved in drug seeking behavior and won't have withdrawal symptoms. There are risks of side effects, drug interactions, and even overdose with both methadone and buprenorphine, which is why they are highly controlled and can only be prescribed in a clinical setting.

Buprenorphine and methadone clients take these medications regularly so they don't experience withdrawal symptoms, etc. but ideally at a dose which isn't at the same time making them feel euphoria. Euphoria is possible at at higher dosages, more significantly with methadone than buprenorphine. At programs which treat methadone and buprenorphine clients only, treatment therapies are generally limited beyond managing the medication and the primary treatment objective is to address the physical symptoms of their dependence. Some clinics do provide a limited amount of counseling and group therapy as secondary services, and it is recommended and encouraged.
