There are substance abuse programs offered in or near San Juan which service both methadone and buprenorphine clients only, controlled medications which are administered in what is called opioid maintenance therapy. Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, meaning the same neural receptors that make a person feel high from morphine or heroin are stimulated by these two drugs so therefore an opiate addicted client will ideally no longer engage in drug seeking behavior and won't experience withdrawal symptoms. There are dangers with side effects, drug interactions, and even addiction and overdose with both buprenorphine and methadone, which is why they are highly regulated and can only be administered in a clinical setting.
Buprenorphine and methadone clients take these medications daily so they don't experience cravings, withdrawal, etc. but ideally at a dosage which isn't also making them feel high. Euphoria occurs at at higher dosages, more significantly with methadone than buprenorphine. At clinics which treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are generally limited beyond managing the medication and the primary treatment objective is to handle the physical symptoms of their dependence. Some centers do deliver a limited amount of counseling and therapy as auxiliary services, and this is recommended and encouraged.
