The Substance Abuse and Mental Health Services Administration, "SAMHSA" for short, is the government department in charge of efforts to promote the mental health of the nation. One of its responsibilities is the supervision of opioid treatment facilities, which are programs which provide medication-assisted treatment (MAT) to individuals who are experiencing opioid dependency and addiction. Drug prescribed are drugs such as methadone, buprenorphine and other drugs which help withdrawal and curb cravings for opioid addicted clients. SAMSHA certified opioid rehabilitation programs in Appalachia are programs that are legally allowed to distribute the medicines used in MAT as long as they remain certified and always maintain the necessary requirements. A requirement that MAT clients must also get auxiliary services in addition to medication like counseling or behavioral therapy.

An opioid rehab program can receive basic approval documentation while working towards becoming fully accredited, however, are required to get their full accreditation in a year. Opioid rehab facilities accredited by SAMSHA need to get licensed also by their own state. Likewise, they need to list themselves with the Drug Enforcement Administration via a local DEA branch. Once an opioid rehab program is approved to operate, it must be re-certified each year or every three years contingent upon the type of accreditation that was given.
