The Substance Abuse and Mental Health Services Administration, "SAMHSA" for short, is the government agency responsible for efforts to advance the behavioral health of the United States. One of its responsibilities is the oversight of opioid rehabilitation facilities, which are centers that offer medication-assisted treatment (MAT) to clients who are experiencing opioid addiction and dependency. Medications given are drugs such as methadone, buprenorphine and other drugs that stop curb cravings and withdrawal for opioid dependent individuals. SAMSHA certified opioid rehab centers in Big Island are programs who are legally permitted to distribute the medicines used in MAT as long as they remain certified and always maintain the necessary requirements. One of these requirements is that MAT individuals must also receive auxiliary services in addition to medicine such as psychotherapy, behavioral therapy or counseling.

An opioid treatment center can receive initial approval documentation while working towards becoming fully accredited, but must become fully accredited within a year. Opioid rehabilitation programs authorized by SAMSHA need to get licensed also by their own state. In addition, they must register with the Drug Enforcement Administration through a local DEA office. Once an opioid rehabilitation center is certified, they need to get re-certified each year or every 3 years depending on the kind of certification that was given.
