There are substance abuse programs available in or near Berlin which service both methadone and buprenorphine clients only, medications that are used in what is known as opioid maintenance therapy. Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, meaning the same neural receptors that cause an individual to feel high from heroin or prescription pain killers are stimulated by these two drugs so that an opioid dependent client will ideally no longer become involved in addict behavior and won't experience cravings and withdrawal. There are dangers with drug interactions, side effects, and even addiction and overdose with both methadone and buprenorphine, which is why they are highly controlled and can only be dispensed in a clinical setting.
Methadone and buprenorphine clients take these medications daily so they don't experience cravings, withdrawal, etc. but ideally at a dosage which isn't also causing them to feel high. Euphoria is possible at at higher dosages, more so with methadone than buprenorphine. At programs that treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are limited beyond managing the medication and the primary treatment objective is to treat the withdrawal symptoms and put an end to the illicit drug use. Some programs do provide a small amount of counseling and group therapy as supplementary services, and this is encouraged.
