Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, which means the same neural receptors that make a person feel high from morphine or heroin are stimulated by these two drugs so therefore an opiate dependent client will instead no longer engage in drug seeking behavior and won't have cravings and withdrawal. There are risks of side effects, drug interactions, and even overdose with both methadone and buprenorphine, which is why they are heavily controlled and can only be dispensed in a clinical setting.

Buprenorphine and methadone clients are on these medications daily so they don't experience cravings, withdrawal, etc. but hopefully at a dosage which isn't at the same time causing them to feel high. Euphoria is possible at at higher dosages, more so with methadone than buprenorphine. At clinics that treat methadone and buprenorphine clients only, treatment therapies are limited beyond this and the primary treatment goal is to manage the physical symptoms of their dependence. Some clinics do provide a limited amount of counseling and group therapy as auxiliary services, and it is encouraged.
