Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, which means the same nerve cell receptors that make a person feel high from morphine or heroin are activated by these two drugs so therefore an opiate addicted client will ideally not engage in drug seeking behavior and won't experience cravings and withdrawal. There are dangers with drug interactions, side effects, and even addiction and overdose with both buprenorphine and methadone, which is why they are highly regulated and can only be prescribed in a clinical setting.
Buprenorphine and methadone clients take these medications regularly so they don't experience withdrawal symptoms, etc. but ideally at a dose which isn't at the same time causing them to feel euphoria. Euphoria can be experienced at at higher dosages, more so with methadone than buprenorphine. At clinics which treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are limited beyond this and the main treatment objective is to manage the physical symptoms of their dependence. Some clinics do deliver a limited amount of counseling and therapy as secondary services, and it is recommended and encouraged.
