Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, which means the same nerve cell receptors which make a person feel high from morphine or heroin are stimulated by these two drugs so that an opiate dependent client will ideally not become involved in addict behavior and won't have withdrawal symptoms. There are dangers with drug interactions, side effects, and even overdose with both buprenorphine and methadone, which is why they are highly regulated and can only be prescribed in a clinical setting.
Buprenorphine and methadone clients are on these medications consistently so they don't experience cravings, withdrawal, etc. but ideally at a dose which isn't also making them feel high. Euphoria is possible at a high dosage, more so with methadone than buprenorphine. At centers that treat methadone and buprenorphine clients only, treatment therapies are limited beyond this and the main treatment objective is to handle the withdrawal symptoms and put an end to the illicit drug use. Some clinics do provide a limited amount of counseling and group therapy as supplementary treatment, and it is encouraged.
