Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, meaning the same nerve cell receptors which make a person feel high from morphine or heroin are stimulated by these two drugs so therefore an opioid dependent client will instead not become involved in drug seeking behavior and won't have withdrawal symptoms. There are dangers with side effects, drug interactions, and even addiction and overdose with both buprenorphine and methadone, which is why they are highly controlled and can only be dispensed in a clinical setting.

Buprenorphine and methadone clients take these medications daily so they don't experience withdrawal symptoms, etc. but ideally at a dosage which isn't at the same time causing them to feel euphoria. Euphoria can be experienced at a high dosage, more so with methadone than buprenorphine. At centers that treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are limited beyond managing the medication and the main treatment objective is to manage the physical symptoms of their dependence. Some centers do deliver a limited amount of counseling and therapy as auxiliary services, and this is recommended and encouraged.
