Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, meaning the same nerve cell receptors which make a person feel high from heroin or prescription pain killers are activated by these two drugs so therefore an opiate addicted client will ideally not engage in drug seeking behavior and won't experience withdrawal symptoms. There are risks of side effects, drug interactions, and even addiction and overdose with both methadone and buprenorphine, which is why they are heavily regulated and can only be prescribed in a clinical setting.
Methadone and buprenorphine clients take these medications regularly so they don't experience withdrawal symptoms, etc. but hopefully at a dose which isn't at the same time causing them to feel high. Euphoria is possible at a high dosage, more so with methadone than buprenorphine. At clinics which treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are limited beyond this and the primary treatment goal is to handle the withdrawal symptoms and put an end to the illicit drug use. Some programs do offer a limited amount of counseling and group therapy as supplementary treatment, and it is recommended and encouraged.
