Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, meaning the same neural receptors that make a person feel high from morphine or heroin are activated by these two drugs so that an opioid addicted client will ideally no longer engage in drug seeking behavior and won't have withdrawal symptoms. There are dangers with side effects, drug interactions, and even overdose with both buprenorphine and methadone, which is why they are heavily controlled and can only be prescribed in a clinical setting.

Buprenorphine and methadone clients take these medications regularly so they don't experience cravings, withdrawal, etc. but ideally at a dose which isn't at the same time making them feel high. At centers that treat methadone and buprenorphine clients only, treatment therapies are limited beyond managing the medication and the primary treatment objective is to manage the physical symptoms of their dependence. Some programs do provide a small amount of counseling and group therapy as supplementary services, and it is encouraged.
