Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, which means the same neural receptors which make a person feel high from heroin or prescription pain killers are stimulated by these two drugs so that an opiate dependent client will instead no longer become involved in addict behavior and won't have cravings and withdrawal. There are risks of side effects, drug interactions, and even addiction and overdose with both buprenorphine and methadone, which is why they are heavily controlled and can only be dispensed in a clinical setting.

Methadone and buprenorphine clients take these medications regularly so they don't experience cravings, withdrawal, etc. but hopefully at a dosage which isn't also causing them to feel euphoria. Euphoria can be experienced at at higher dosages, more significantly with methadone than buprenorphine. At centers which treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are generally limited beyond managing the medication and the primary treatment goal is to address the withdrawal symptoms and put an end to the illicit drug use. Some programs do offer a limited amount of counseling and group therapy as auxiliary treatment, and this is recommended and encouraged.
