Methadone is a full opioid agonist the same as heroin, and buprenorphine is a partial agonist, meaning the same neural receptors which make a person feel high from heroin or prescription pain killers are activated by these two drugs so that an opiate addicted client will ideally no longer engage in drug seeking behavior and won't experience cravings and withdrawal. There are risks of side effects, drug interactions, and even overdose with both methadone and buprenorphine, which is why they are heavily controlled and can only be administered in a clinical setting.

Methadone and buprenorphine clients take these medications consistently so they don't experience cravings, withdrawal, etc. but ideally at a dose which isn't also causing them to feel euphoria. Euphoria is possible at a high dosage, more significantly with methadone than buprenorphine. At clinics that treat methadone and buprenorphine clients only, treatment therapies are limited beyond this and the primary treatment goal is to address the physical symptoms of their dependence. Some centers do provide a limited amount of counseling and therapy as secondary treatment, and it is encouraged.
