There are substance abuse programs offered in or near Smithfield that cater to both methadone and buprenorphine clients only, drugs that are administered in what is called opioid maintenance therapy. Methadone is a full opioid agonist just like heroin, and buprenorphine is a partial agonist, meaning the same neural receptors that make a person feel high from morphine or heroin are stimulated by these two drugs so that an opiate dependent client will instead no longer become involved in drug seeking behavior and won't experience cravings and withdrawal. There are risks of drug interactions, side effects, and even addiction and overdose with both methadone and buprenorphine, which is why they are highly regulated and can only be prescribed in a clinical setting.

Buprenorphine and methadone clients take these medications daily so they don't experience cravings, withdrawal, etc. but ideally at a dosage which isn't at the same time making them feel euphoria. At clinics which treat methadone and buprenorphine clients only, actual rehabilitation activities and therapies are generally limited beyond this and the main treatment goal is to handle the physical symptoms of their dependence. Some clinics do offer a limited amount of counseling and group therapy as auxiliary treatment, and it is encouraged.
